Our wellbeing co-ordinators help patients to get the right support. Many different patients will be referred to wellbeing co-ordinators. They include:
- People with long-term conditions
- Low level mental health issues
- Those who feel lonely or isolated
Wellbeing co-ordinators also help people with complex social needs.
Generally, patients speak with a wellbeing co-ordinator 6-12 times over a three month period. They will connect them with activities, groups, and services in their community. This is for practical and emotional support to help them get back on their feet.
